The Office of Institutional Research and Effectiveness complies with Federal Laws and Regulations associated with the release of institutional data. Upon submission of this form, the requester will be notified of their status. We kindly ask individuals to provide as much notice as possible and be detailed when describing your specific needs. The requester will receive a carbon copy form via e-mail upon submission.

Please try to give us at least 2 weeks - your data may be ready sooner depending on the complexity of the request and other priorities in our office.
